Job Description
Licensed Practical Nurse 7pm - 7am - Rotate every other weekend off

Supports the Registered Nurse and Physician in the provision of resident care, to include delivery of resident care, collection of on-going resident response to care, admissions and discharge planning and resident/family education.
The LPN works in close collaboration with the RN or a licensed physician as well as the Care Plan Team to evaluate the resident’s plan of care and progress toward goal attainment.
