Under limited supervision, performs program duties to include case coordination for individuals with developmental disabilities or mental illness, planning and implementing activities to ensure that the needs of program individuals are met, and insuring compliance with applicable standards. Also may oversee daily activities of Direct Care Staff.

Education and/or Experience Bachelor’s Degree in Human Services or related field preferred or High School Diploma or GED and three to five years of experience in services to persons with developmental disabilities or any equivalent combination of education or experience. Minimum of one year previous supervisory experience required.
Essential Duties and Responsibilities: Other duties may be assigned as necessary.
As directed/determined by the Program Director, schedule and attend Service Plan/Plan of Care meetings, planning meetings, LME provider meetings (only in extreme cases when Program Director is unavailable), Interagency Council, and other meetings as scheduled.
Coordinate objectives and meet deadlines for the implementation of Service Plans/Plans of Care.
Consult with QDDP/QMHP or Program Director regarding performance issues; communicate potential issues involving individuals or staff and case management needs to QDDP/QMHP or Program Director.
Communicate issues or concerns regarding individuals with appropriate team members; communicate with clinical consultants, case managers and families as needed.
Monitor compliance with program and production expectations.
Implement and monitor all state regulations.
Enforce policies, procedures and general guidelines of LIFESPAN.
Ensure that community inclusion and choices are integral parts of the program.
Ensure staff adheres to LIFESPAN Philosophy of Positive Communication, Positive Relationships And Positive Supports.
Ensure programming includes daily living skills training.
Utilize and maximize community resources by attending networking functions and appropriate functions as assigned by the Program Director.
Serve as an advocate for the individuals we serve.
Perform/assist with parts of various problem solving duties which occur, including studying the situation, developing possible solutions, investigating options/resources, deciding on most appropriate plan for solution, and implementing approved plan of action.
Review and conduct staff performance reviews in conjunction with Program Director.
Serve on Program and/or District Health and Safety committee as requested.
Submit payroll, mileage, and expenses in absence of Program Director.
Provide training to other employees via demonstration, repetition, modeling, and large group training, etc.
Communicate and establish good relationships with a variety of individuals and groups, including individuals we serve, supervisors, families/guardians, consultants, case managers, psychologistes, therapists, etc.
Establish and maintain effective relationships with other staff, participants, caregivers, case managers, consultants and community resources.
Develop, monitor and modify daily schedules.
Develop and implement objectives to ensure that individual goals are being done properly.
Oversee data collection.
Cross-train staff on individual issues as needed.
Assist with the coordination of new admissions and individual transfers.
Model positive interactions; adhere to the principles of the LIFESPAN ChOICES Training Curriculum.
Interview and select staff as delegated by Program Director.
Assist with staff disciplinary action, as needed.
Assist with and/or oversee medication administration.
Assist with and/or manage facility maintenance.
Assist individuals with personal hygiene as needed (e.g. assisting with toileting, diapering, feeding, etc).
Assist with community outings and other special events as needed.
Participate in and prepare for audits, if requested. Serve as backup coverage for direct care staff when necessary. Abide by, enforce and participate in the implementation and ongoing oversight of all LIFESPAN safety standards and regulations.
