Job Description Duties: Promote and preserve the health and independence of clients in their home setting. Follow Medicare regulations to evaluate, plan and treat eligible patients in their home setting.
Responsibilities Include: o Assess client in home setting to determine the client’s physical, psychological, environmental, safety and functional needs. o Receives referrals concerning client need for home care services. o Develops and implements a plan of treatment in collaboration with the client, family and physician based on the specific program criteria, all pertinent diagnoses, medications, treatments and functional assessments. Implementation includes monitoring, performing skilled nursing procedures, teaching, making interdisciplinary and community agency referrals and communicating with practitioner and other health professionals. Referrals to home health aide services include aide care planning and supervision. o Evaluates and updates client plan of care and coordinates services from time of admission to time of discharge. o Administers therapeutic treatments, drugs and parenteral solutions as ordered by client’s practitioner. o Maintains legal medical record of care planning, client progress, therapeutic and nursing interventions, and patient responses. o May supervises home health aides as assigned. o Participates on the agency’s Professional Advisory Committee. The committee will determine areas of participation. o Functions within the limits of the Alaska State Nurse Practice Act, mission statement of the Medical Center, and hospital and departmental policies. Follows the Professional Code of Ethics. o Maintains confidentiality of client and Hospital matters. o Presents hospital and Home Health Agency in a positive manner.
Qualifications: Experience: Two years of experience preferred with experience with the elderly and work in geriatrics Autonomy, critical thinking skills are required. Motivated team approach. Licensing: Registered Nurse or Licensed Practical Nurse in the State of Alaska. Also required, Alaska state driver’s license and insured personal vehicle. Other: Excellent communication techniques and a demonstration of the nursing process.
