Medicaid Service Coordinator



Job Description
We are currently recruiting for a Medicaid Service Coordinator. This position will be 40 hours per week, Monday-Friday 9:00am-5:00pm, with flexibility in work schedule as needed to assist the individuals we support.

Medicaid Service Coordinators promote choice, individualized services and supports, and consumer satisfaction for individuals and their families. They develop and monitor the service plan (ISP) involving the active partnership of all those who support the person. They work to foster community connections for the individual/family as reflected in their wishes and needs as well as managing recording/reporting requirements as required.

Salary: Lower $30K's

Education/Requirements:
Minimum of a Bachelor's Degree required and experience working with the developmentally disabled. At this time, we cannot substitute an Associate's Degree in lieu of a completed Bachelor's Degree. Degree coursework must include 21 credit hours in the field of psychology, human services, sociology, or other related social services field.
Candidates must have a valid driver’s license with at least 1 year of driving experience and reliable transportation.
