Forensic Continuity of Care Coordinator/ Forensic Fidelity Manager



Job Description
Education and/or Experience:
• Master’s Degree in Social Work, Sociology, Psychology or Counseling with 2 years Human Services Experience
• Or equivalent

Certificates, Licensure, Registrations:
• Current driver’s license and vehicle for use while at work
• Licensed Professional Counselor, Licensed Clinical Social Worker, Licensed Marriage and Family Therapist preferred

Position Summary:
To advocate for and to assist individuals admitted to the Forensic program with coping in crisis situations and with admission into services for those who are referred by the criminal justice system and diagnosed with severe and persistent mental illness. To provide assessment, diagnosis, counseling and treatment services to identified Forensic clients. Develop and implement HHSC/TAC approved training methodologies for TCC employees who work within the Forensic program. Monitor and evaluate Adult Mental Health service documentation submitted by Forensic staff to ensure all HHSC and CMS and Forensic requirements are met or exceeded. Attend the regularly scheduled Fore audits and client reviews. Assist Program Directors with community outreach and trainings when necessary. Monitor and evaluate service documentation submitted by QMHP staff to ensure all regulatory requirements are met or exceeded. Provide Utilization Management oversight and authorization for documentation, including authorization for Medical Necessity. Other duties as requested and authorized by Forensic Program Director in accordance with the philosophy, goals, and objectives of Texoma Community Center. Starting wage between $47,000 to $49,000.
