ARCO, A Community Resource, is seeking a dedicated and compassionate Service Plan Coordinator to join our team. As a nonprofit United Way agency serving adults with developmental disabilities and their families throughout Northeast Louisiana since 1954, ARCO is committed to helping individuals achieve greater independence, inclusion, and quality of life.
The Service Plan Coordinator plays a vital role in ensuring high-quality, person-centered services by assessing participant needs, developing and monitoring service plans, coordinating with families and community partners, and promoting compliance with agency, state, and federal requirements. This position also provides leadership and supervision to Plan Coordinators, assists with quality assurance activities, reviews Critical Incident Reports (CIRs), and collaborates with program leadership to continuously improve service delivery.

Essential Functions
Participant Assessment and Service Planning
- Collaborate with participants, families, legal representatives, advocates, and ARCO staff to identify participant strengths, needs, preferences, interests, and available resources.
- Work with Team Managers, Finance, Vocational Services, Day Program Managers, and other staff to develop strategies and supports that address participants' long-term and day-to-day needs.
- Conduct comprehensive assessments and maintain current participant records.
- Develop, implement, and monitor Plans of Care and related service documentation.
- Collaborate with Department Directors, Team Managers, and other staff to develop goals, budgets, and schedules for each Comprehensive Plan of Care (CPOC).
- Evaluate the effectiveness of service plans and identify areas requiring improvement.
- Notify appropriate leadership staff when unmet needs are identified and facilitate corrective action plans.
- Monitor progress of improvement plans and provide updates to the administrative team until issues are resolved.
- Coordinate with external service coordinators and the regional NEDHSA Waiver Unit regarding waiver resources and participant services.
- Attend annual planning meetings, quarterly reviews, revisions, and other meetings necessary for effective service coordination.
- Advocate for participant needs with funding agencies, service coordinators, families, and community partners.
- Work with the Billing Director to monitor authorized service units and submit revisions as needed.
- Submit, review, and evaluate Critical Incident Reports (CIRs) and make recommendations to Department Directors and Team Managers.
- Assist with the collection, tracking, analysis, and reporting of quality assurance and quality enhancement data.
- Collaborate with the Staff Development Director to identify training needs and provide staff training or coaching as necessary.
- Ensure compliance with agency policies, Medicaid waiver requirements, and all applicable state and federal regulations.
- Ensure completion of 90-L documentation and other eligibility requirements necessary for waiver participation.
- Conduct intake assessments for new Supported Living and Personal Care Attendant (PCA) participants and assist with admissions to other programs as requested.
Minimum Qualifications
All applicants must:
- Be at least 21 years of age.
- Possess a current, valid driver's license.
- Have dependable transportation available for work-related travel.
- Provide proof of automobile insurance, vehicle registration, and inspection.
- Meet ARCO's insurability requirements.
- Provide six (6) business references.
- Provide three (3) personal references.
- Successfully pass all required background screenings, including those required by the Centers for Medicare & Medicaid Services (CMS).
- Be able to perform the essential functions of the position with or without reasonable accommodation.
