Job Summary
We are currently recruiting for a Transportation Driver/CNA to join our Haven Nursing Center team! The person filling this position must be a Certified Nursing Assistant (CNA) in the State of Louisiana with a valid driver's license and a clean driving record.
Residents at Haven Nursing Center live in units referred to as households. The Transportation Driver/CNA is an essential member of the household team and plays a vital role in ensuring residents receive safe, timely transportation to medical appointments while also providing direct resident care within the facility.
The Transportation Driver/CNA is responsible for transporting residents to physician offices, hospitals, dialysis, therapy appointments, clinics, and other approved destinations. When transportation responsibilities are complete, the employee is expected to assist nursing staff by providing resident care and performing assigned duties within the scope of CNA practice.
This position is primarily scheduled Monday through Friday from 8:00 a.m. to 4:30 p.m. However, appointment schedules vary, and the employee must be available to work earlier or later as needed to accommodate resident transportation needs. Flexibility is an essential requirement of this position.
Primary Duties
Transportation Responsibilities
- Safely transport residents to and from physician appointments, hospitals, dialysis centers, clinics, and other approved destinations.
- Complete multiple transportation trips throughout the day based on resident appointment schedules.
- Assist residents with entering and exiting the transportation vehicle using proper transfer techniques.
- Safely secure residents and mobility equipment during transport.
- Ensure residents arrive safely and on time for scheduled appointments.
- Remain with residents during appointments when necessary.
- Return physician documentation, appointment summaries, and new orders to the nursing department promptly.
- Complete daily vehicle inspections and report maintenance or safety concerns immediately.
- Maintain transportation logs, mileage records, and vehicle cleanliness.
- Follow all traffic laws and operate the facility vehicle in a safe and professional manner.
Resident Care Responsibilities
- Assist residents with activities of daily living (ADLs).
- Help residents with mobility, transfers, positioning, and ambulation.
- Monitor and report changes in residents' condition to licensed nursing staff.
- Obtain and record vital signs and other basic health information as assigned.
- Respond promptly to call lights and resident needs.
- Maintain a clean, safe, and organized resident environment.
- Assist with meal service and hydration needs when assigned.
- Provide emotional support, encouragement, and respectful companionship to residents.
- Follow individualized care plans, infection control standards, and facility safety procedures.
- Document care provided accurately and timely.
Team Responsibilities
- When transportation duties are complete, assist coworkers on the nursing units with resident care and other assigned duties.
- Follow directions from supervisors and willingly assist in areas where additional help is needed.
- Maintain a positive attitude and contribute to a team-oriented work environment.
- Represent Haven Nursing Center professionally while interacting with residents, families, medical providers, and the public.
Minimum Qualifications
- Commitment to person-centered care.
- Current Louisiana CNA certification.
- Valid Louisiana driver's license.
- Clean driving record.
- Must be at least 18 years of age.
- Ability to safely operate a passenger van.
- Ability to lift, move, and assist residents safely.
- Strong communication, customer service, and teamwork skills.
- Compassionate, patient, dependable, and professional demeanor.
- Flexible availability to accommodate changing resident appointment schedules.
Physical Requirements
- Must be able to lift, push, or pull up to 50 pounds.
- Must be able to lift, carry, stand, bend, squat, reach, kneel, and assist residents using proper body mechanics for sustained periods.
- Must be able to safely load, unload, and secure wheelchairs and mobility equipment.
- Must be able to sit for extended periods while driving and frequently enter and exit the transportation vehicle.
Additional Requirements
- Active CNA certification in the State of Louisiana.
- Valid Louisiana driver's license with an acceptable driving record.
- Successfully pass a criminal background check.
- CPR Certification (or willingness to obtain prior to employment).
- Tuberculosis screening (or chest x-ray) required prior to employment.
- Employee reference checks will be conducted and may include a review of job performance and contact with previous employers.
