🩺 What You'll Do
- Complete skilled nursing assessments and visits
- Provide wound care and other ordered treatments
- Assist with medication management and patient education
- Monitor patient progress and communicate changes
- Coordinate with physicians and the interdisciplinary care team
- Complete accurate and timely documentation
What We're Looking For
- Current Registered Nurse (RN) license
- Valid driver's license and reliable transportation
- Strong clinical and communication skills
- Dependable, compassionate, and able to work independently
- Home health experience preferred
