Job Description

Cherokee County Health and Rehabilitation Center is seeking a full time MDS Assistant.

- Monday - Friday 7:00a to 3:30pm
- Must be available to work some weekends or holidays if needed
- Must have a current Alabama LPN or RN license and be up to date with MDS certifications

The MDS Coordinator is to provide leadership to our skilled nursing facility in the management of the Minimum Data Set (MDS) and utilization management processes in accordance with all current federal, state and local standards, guidelines and regulations and the facilities established policies and procedures. To assure compliance with regulatory agencies and generally accepted clinical standards, responsible for coordinating, recording, updating, and monitoring all Minimum Data Set documentation required by state and other appropriate authorities.

The primary purpose of the MDS Coordinator is to record, update and monitor all MDS documentation required by state and other appropriate authorities. To develop and coordinate nursing care plans from MDS/RUG category, analyze assessment data independently for complex resident problems and identify early subtle changes that formulate RUG categories and effect resident care. Electronically transmit the data in a timely manner and communicate effectively with other members of the healthcare team to create and carry out resident healthcare plans.
