Registered Nurse for PACE

Job Description

Immediate need for a Registered Nurse for Mountain Empire Older Citizens Program of All Inclusive Care for the Elderly (PACE).

RN will provide direct and indirect nursing care to geriatric patients in the medical clinic, triage after-hours calls - rotating call schedule. Administers medications, assessments, changes dressing, provides wound care as well as being a liaison between providers and patients. Documents in EHR patient care delivery, condition, progress in a timely manner. Provides patient education to patients and family members. Clinic hours are 8:30 a.m. – 4:30 p.m., Monday-Friday. After-hours call rotation. Competitive salary and benefits.


QUALIFICATIONS:

• Graduate of an accredited school of nursing and be licensed by the Virginia Board of Nursing as a registered nurse.
• Must have practiced nursing in the last three years.
• One or more years experience with a geriatric population
• Ability to manage people in stressful, difficult and unusual situations.
• Medically cleared for communicable diseases with up-to-date immunizations before engaging in direct participant contact.
• Criminal history records check at hire.
• Drug screen at hire.

The nurse will join a clinical team that includes nurses, one NP and one MD. The medical team is part of a larger interdisciplinary group that works together to care for older adults with chronic illnesses who live at home and need help with activities of daily living.

Along with the physician, nurse practitioner and registered nurse, the interdisciplinary team includes a dietician, pharmacist, social worker, physical therapist, occupational therapist, home care coordinator, transportation specialist, recreation director and PACE center manager.
