Case Manager - Stamping Lofts

Job Description

The Case Manager, as part of the Stamping Lofts Housing Program and Project LIVE (Living Independently, Valuing Empowerment), will provide
comprehensive continuum of services and individualized case management to address barriers to housing stability. The
Case Manager will also provide in-home assistance and regular coaching through ongoing individual case management

• Maintain minimum of 56 qualified applicants into program annually
• Conduct informational group sessions about Stamping Lofts Housing Program
• Provide supportive services to all residents
• Assist residents with obtaining employment
• Assist individuals exiting Stamping Lofts with housing search via internet and in community (includes transporting program participants)
• Network in community monthly to obtain additional resources related to housing, employment, utility assistance, educational opportunities, etc.
• Assist residents with obtaining SSI/SSDI entitlement benefits through SOAR program model
• Conduct home visits weekly with residents
• Assist participants with ADL’s
• Complete ongoing assessments to determine client needs and develop Housing Stability Plans
• Complete monthly budget planning with residents.
• Develop, update and track self-sufficiency goals monthly relating to housing and overall well-being
• Assist individuals in crisis by providing direct services and or referrals
• Receive and document all additional information needed for hard copy file
• Participate in team staffing to discuss progress toward goals and self-sufficiency
• Receive and document all data needed for monthly and annual reporting in HMIS systems.
• Conduct Housing Quality Standards Inspections annually and as needed for repairs
• Coordinate services across SPC programs
• Update all appropriate CASE WORTHY HMIS categories for each client seen within 24 hour period of providing services
• Provide in-home Living Skills coaching as needed
• Assist and complete special case management projects as directed by Program Manager and Senior Director

• Bachelor degree in Social Work or related field preferred
• Knowledge of Housing First Model
• Minimum of two years-experience working with low-income or homeless individuals/families
• Minimum of two years-experience working with chronic homeless population
• Basic knowledge of HUD Programs
• Experience with wrap-around services preferred
• Experience in conducting home visits
• Daily charting
• SOAR certification preferred

• Understanding and empathy for the needs and concerns of low-income, chronic homeless, chemically dependent and individuals diagnosed with mental health conditions
• Possess multi-tasking abilities
• Excellent listening skills
• Excellent spoken and written communication skills
• Ability to analyze problems and make sound decisions
• Excellent organizational skills
• Ability to demonstrate creativity in client engagement
• Ability to use Microsoft Office computer software
• Ability to work independently as well as in team setting
