Weekend Case Manager

Job Description

The Weekend Case Manager will assist in implementing the daily direct care of all clients/residents in compliance with treatment and program goals for Rosati Center Group Home.

RESPONSIBILITIES
• Maintain a positive relationship with each resident
• Assist in implementation of treatment goals by knowing the diagnosis, medications and history of each client/resident
• Conduct comprehensive, ongoing assessments of mental illness, substance use, physical health, housing status and other relative needs of each client/resident
• Provide direct clinical services on an individual and group basis in office and community setting by making appropriate changes to treatment plans to ensure abilities and resources of each client/resident
• Assist in creating a comfortable atmosphere for clients/residents
• Treat each client/resident with respect and keep good professional boundaries
• Report behavioral/physical changes, problems and/or concerns regarding residents to daytime Social Worker and respond/intervene as needed in a therapeutic manner
• Maintain Group home client case notes through BJC requirements and Case Worthy
• Direct incoming calls in a timely and professional manner at the Front Desk
• Provide crisis intervention when necessary
• Communicate regularly with supervisor for performance and feedback as well as special assignments
• Perform any and all miscellaneous tasks as required or requested by Supervisor, Social Worker and/or management
• Other miscellaneous and special projects as assigned

KNOWLEDGE AND EXPERIENCE
• Master's Degree in Social Work or Counseling required
• Understanding of mental illness and substance use
• Experience in counseling and case management
• High level of confidentiality, sound judgment and discretion
• Excellent written and verbal communication skills
• Solid computer skills
• Must be detail oriented, accurate and reliable
• Ability to multi task in a fast-paced team setting and ability to work independently
• Knowledge/understanding of area/community resources to provide appropriate referrals
• Knowledge of Federal, State and Agency policies of HIPPA confidentiality

RELATIONSHIPS REQUIRED
• Report to Director of Rosati Center
• Attend and participate in all appropriate St. Patrick Center meetings
