Works collaboratively with the Director of Home Health, Advisory Board and Administration in assisting the Director in providing administrative direction for the provision of home health services.
Maintains effective and efficient use of resources including facilities, staff, equipment, time, capital and inventory through support of quality improvement processes.
Assists the Director in financial planning and control and nursing standards.
Accountable for employee performance review and quality assurance for all Nursing and Therapy Staff.
Completes admission assessments, initiates patient plans of care, completes Oasis forms as required, responsible for coordinating all patient care for assigned team.
Responsible for updating patient plan of care through re-certification at least every 60 days (if patient to continue on the program for longer than 60 days) following Taylor Home Health Care Policies and Procedures.
Prepares/conducts Case Conferences for patients on pertinent patients.
Prepares and reviews daily SN schedule, makes changes and notifies Nursing Staff of changes as necessary, checks Nursing Staff in and out each day in order to assure proper communication of schedule changes, scheduling PRN SN visits if needed.
Responsible for coordination of services with Therapy services to ensure proper communication and scheduling of new referrals as well as noted changes in patient condition/plan of care.
Takes telephone referrals following relevant policies and procedures.
Primary coordinator of review of patient lab results and reporting results to MD.
Maintains Coumadin log and ensures proper scheduling of all labs and procedures as changes in patient plans of care occur.
Receives verbal orders for patients as needed and coordinates changes with all members of the Health Care Team.
Directly responsible for supervision of licensed Nursing Staff.
Provides direction and instruction to licensed Nursing Staff as needed. Initiates disciplinary process for Nursing Staff as indicated.
Performs employee evaluations for licensed Nursing Staff annually and forwards to Director for review prior to forwarding to Human Resources.
Responsible for competence assessment annually and as needed.
Responsible for annual skills check off.
Performs clinical record review/utilization review activities.
Provides input as to documentation, utilization of visits, care plan standards and quality of care.
Works with the Director/Nursing Supervisor on quality of care and documentation.
Reports problems concerning patients to the appropriate team members as necessary.
Assists with resolution through discussion of problems.
Reviews Skilled Nursing notes and maintains clinical record documentation to meet regulations and guidelines as specified by State and Federal agencies.
Informs staff of changes in Medicare and Medicaid guidelines.
Completes additional orders as needed to ensure regulatory guidelines are met.
Maintains proper utilization of services and documentation of continuing need for services.
Coordinates continuing Education material and meetings for clinical nursing staff in conjunction with the Director.
Works in conjunction with Director to encode Oasis assessments as required.
In absence of Director, assists Office Manager with preparing payroll, encoding OASIS documents; reviewing re-certifications and all Physician orders before being keyed.
Attends OBQI meetings, updates Taylor Home Health Care procedures as needed in order to assure quality patient care and diligently works to achieve OBQI related goals.
Attends/assists Director in coordination of Staff Meetings, In-services and Advisory Board meetings.
Continually strives to improve knowledge through attendance at workshops, in-services, professional and related organizations, individual research and reading.
Responsible for surveillance and prevention of both staff and employee infections.
Assists registered nurses with admissions or visits in times of high census or other critical needs.
RN Required (BSN Preferred), Current Licensure, Prior Home Health Supervisory Experience Required, EMR/EHR Experience, Clinical Skills, Bedside Manner, Infection Control, Nursing Skills, Physiological Knowledge, Administering Medication, Medical Teamwork, Multi-tasking, Listening, Verbal Communication, Health Promotion and Maintenance
