Texoma Community Center (TCC) is one of 39 Texas Community Centers, which are governmental entities as defined by Title 7 of the Texas Health and Safety Code. Salary $17.57 an hour.

This job primary functions of this position include ensuring that our consumers are connected with the services that they need and for collaborating with staff across the agency and with external agency partners. The Care Coordinator provides continuity of care patient, impact monitoring and service assessment provision and monitoring. Provides service coordination and brief interventive treatment to clients of all ages with behavioral health issues who have been referred to TCC, received any TCC services, and/or discharged from psychiatric hospitals to TCC’s catchment area. Implement and process referrals for TCC and other community services while also working closely with other team members to monitor data and activities related to 1115 Waiver, PPB, and CCBHC requirements. Other duties as assigned by AMH Director in accordance with the philosophy, goals and objectives of Texoma Community Center. Operates under trauma-informed and recovery-oriented models of care which align with CCBHC standards. Other duties as assigned.

Education and/or Experience:
• Bachelor’s Degree in Social Work, Counseling, or Psychology meeting criteria for Qualified Mental Health Professional required
• Master’s Degree in Social Work, Counseling or Psychology preferred
• Experience working with individuals with mental illness and/ or addiction issues preferred
• Local Travel required

Certificates, Licensure, Registrations:
• Current driver’s license and vehicle for use at work
