Texoma Community Center (TCC) is one of 39 Texas Community Centers, which are governmental entities as defined by Title 7 of the Texas Health and Safety Code.
This position provides appropriate linkage to services for individuals with behavioral health issues from nursing facilities and/or seeking continuity of care services. To perform PASRR assessments in nursing facilities in accordance with the HHSC guidelines and provide skills training and case management services to individuals residing in nursing homes who have been identified as having a mental illness based on person centered approach. Provides rehabilitation case management or service coordination services as outlined in the Texas Administration Code and current state guidelines for persons with persistent and severe mental illness including advocacy, crisis services, individual and group skills training in accordance with the philosophy, goals and objectives of Texoma Community Center. Operates under trauma-informed and recovery-oriented models of care which align with CCBHC standards. Starting wage is $14.71 + incentive.
Education and/or Experience:
• Bachelor’s Degree in Social Work, Sociology or Psychology required. Master’s Degree and two (2) years community mental health experience preferred.
Certificates, Licensure, Registrations:
• Current driver’s license and vehicle for use while at work.
